Patient-responsibility amounts are due before or at the time of service unless otherwise arranged.

Self-pay patients are responsible for payments in full before the completion of their appointment.

Patients using insurance are responsible for any applicable:

·      Copayments

·      Deductibles

·      Coinsurance

·      Non-covered services

·      Outstanding balances

Cancellation & No-Show Policy

To respect the time of all patients and providers, we request at least 24 hours’ notice for cancellations or appointment changes.

Cancellation and no-show fees are not billed to insurance and are the patient’s responsibility, except where prohibited by applicable law or payer requirements.

 Repeated missed appointments may result in dismissal from the practice.
